Effective Date: January 1, 2024 · Last Updated: April 2025
We collect information you provide directly to us, including your full name, date of birth, address, phone number, email address, and insurance information when you schedule an appointment, complete patient forms, or contact us.
As a dental practice, we collect and maintain protected health information (PHI) including your medical and dental history, treatment records, X-rays, prescriptions, and billing information. This information is governed by the Health Insurance Portability and Accountability Act (HIPAA).
When you visit our website, we may automatically collect certain information such as your IP address, browser type, operating system, referring URLs, and pages viewed. This information is used solely to improve our website and services.
Your health information is used to provide, coordinate, and manage your dental care, including scheduling appointments, processing insurance claims, and communicating with other healthcare providers involved in your treatment.
We may use your contact information to send appointment reminders, treatment follow-ups, billing statements, and important notices about our practice. You may opt out of non-essential communications at any time.
We may use or disclose your information as required by law, including in response to court orders, subpoenas, or other legal processes.
As our patient, you have the right to: (1) access and obtain a copy of your health records; (2) request corrections to your health information; (3) request restrictions on how we use or disclose your PHI; (4) request confidential communications; (5) receive an accounting of disclosures of your PHI; and (6) receive a paper copy of our Notice of Privacy Practices.
To exercise any of these rights, please contact our office in writing. We will respond to your request within 30 days. There may be a reasonable fee for copies of records as permitted by law.
Element Family Dental does not sell, rent, or trade your personal or health information to third parties for marketing purposes.
We may share your information with other healthcare providers involved in your treatment, insurance companies for billing purposes, and business associates who help us operate our practice — all under strict confidentiality agreements.
We may disclose information when required by law, to prevent serious harm, or to comply with public health reporting requirements.
We implement administrative, physical, and technical safeguards to protect your health information as required by HIPAA. Our electronic systems use encryption, secure login, and access controls to prevent unauthorized access.
Any information submitted through our website forms is transmitted over encrypted (SSL/TLS) connections. We do not store sensitive health information from web form submissions beyond what is necessary to respond to your inquiry.
Our website may use cookies and similar technologies to improve your browsing experience and analyze website traffic. These cookies do not collect personal health information. You can control cookie settings through your browser preferences.
We provide dental care to patients of all ages, including children. For patients under 18, a parent or legal guardian must provide consent for treatment. Health records for minor patients are handled with the same privacy protections as adult records.
We only send SMS text messages to patients who have affirmatively opted in by checking the SMS consent checkbox on our patient registration form. The checkbox is unchecked by default and requires a deliberate action to enable. Your consent is recorded with a timestamp and stored securely in our HIPAA-compliant system.
If you opt in, you may receive appointment reminders, appointment confirmations, and practice notifications from Element Family Dental via text message. We do not send marketing or promotional messages unrelated to your care.
Upon opting in, you will receive a confirmation text message acknowledging your enrollment. You may opt out at any time by replying STOP to any message you receive from us. After opting out, you will receive one final confirmation that you have been removed from our SMS list and will receive no further messages.
Message frequency varies based on your appointment schedule. Message and data rates may apply depending on your mobile carrier and plan. Element Family Dental is not responsible for any charges incurred from your carrier.
Text messages from Element Family Dental will not contain protected health information (PHI). Messages are limited to scheduling logistics and practice notifications. For clinical matters, please contact our office directly by phone.
We may update this Privacy Policy periodically to reflect changes in our practices or legal requirements. We will post the updated policy on our website with the revised effective date. Continued use of our services after changes constitutes acceptance of the updated policy.
